Below, you’ll find the School Data Collection Form designed for new starters. This form captures essential details about your child, including contact information, background, and medical history. Additionally, it contains our Biometric Consent Form for the in-school catering system (allowing your child to purchase food from the canteen and top up their lunch money in school) and Photographic Consent Form which allows to to use images/video footage taken of students whilst involved in school/Trust events/activities in our school publications; (school & trust websites, social media pages, promotional material, leaflets, books, brochures or other school/trust publications)
